Position Overview

BronxWorks is dedicated to enhancing the economic and social well-being of individuals and families. We are currently seeking a Client Services Coordinator to join our Access to Benefits Program.

Key Responsibilities:
  • Oversee a comprehensive caseload for a varied client demographic.
  • Conduct initial assessments and enter data into our management system.
  • Facilitate both internal and external referrals for clients as necessary.
  • Advocate for clients and provide follow-up services.
  • Maintain detailed documentation of client interactions and progress in the database.
  • Assist clients in completing applications for government benefits and other services.
  • Engage in outreach activities and participate in agency food pantry operations as scheduled.
  • Perform additional tasks as assigned by management.
Qualifications:
  • A Bachelor’s degree in a relevant field is required.
  • Bilingual proficiency in verbal and written communication is preferred.
  • Familiarity with modern business communication tools, including computers and electronic mail.
  • A minimum of two years of experience in case management or a related field is required.
Essential Duties:
  • Deliver direct services to clients, including intake, engagement, assessment, service plan formulation, referrals, advocacy, counseling, and follow-up.
  • Manage an active caseload effectively.
  • Monitor and document client progress towards established service plan objectives.
  • Ensure thorough documentation of all client interactions.
  • Stay informed about available program resources both on-site and off-site.
  • Provide group counseling sessions as directed by the Program Coordinator or Director.
  • Coordinate client services with other social service, mental health, medical, educational, and childcare providers.
  • Conduct regular case conferences with clients and service providers as needed.
  • Prepare program reports on a monthly basis or as required.
  • Meet with supervisors regularly to discuss client progress and program needs.
  • Conduct home visits and fieldwork as necessary.
  • Perform additional duties as assigned.
Physical Requirements:
  • Ability to work on a computer for extended periods.
  • Occasional lifting and/or moving of items up to 10 pounds.
  • Capability to stand, walk, or sit for prolonged periods.
  • Ability to bend and retrieve documents and objects.
  • Willingness to travel within the boroughs and adjacent areas using public transportation.
  • Ability to travel at least 1 mile in various weather conditions for home visits and fieldwork as required.
