Job Overview

We are currently in search of a Case Manager to enhance our RISE Program. This role involves comprehensive case management for clients, alongside meticulous record-keeping for those engaged with the RISE initiative. The Case Manager will report directly to the Program Coordinator.

Compensation: $27.47 per hour

Key Responsibilities:

  • Deliver direct client services, including outreach and assessments via videoconference in correctional facilities, intake processes, engagement, service plan formulation, referrals, advocacy, counseling, exit summaries, and follow-up.
  • Manage a diverse caseload effectively.
  • Track and document client progress towards established service plan objectives.
  • Maintain comprehensive records of all client interactions.
  • Stay informed about available program resources both on-site and off-site.
  • Coordinate client services with various social services, mental health, medical, employment, educational, and other providers.
  • Facilitate case conferences involving clients, service providers, and legal representatives as necessary.
  • Accompany clients to court appearances when required.
  • Prepare concise update letters for the Court regarding client progress.
  • Engage in bi-monthly meetings with the supervisor to report on case management activities.
  • Ensure compliance with all relevant regulations and organizational policies.
  • Provide crisis intervention and stabilization support as needed.
  • In-person collaboration with colleagues is essential.
  • Perform additional duties as assigned.

Qualifications:

  • A Bachelor’s degree in criminal justice, social work, psychology, or a related field is preferred.
  • Alternatively, a High School Diploma or equivalent with a minimum of three years of progressive case management experience in the criminal justice sector.
  • A minimum of three years of relevant case management experience is required.

  • Strong interpersonal skills and attention to detail are crucial.
  • Excellent oral and written communication, time management, and organizational abilities are necessary.
  • Familiarity with community resources is advantageous.
  • Problem-solving, decision-making, conflict resolution, and active listening skills are essential.
  • Ability to remain calm and effective in crisis situations.
  • Proven capability to collaborate effectively with diverse groups.
  • Must be adept at managing multiple tasks efficiently under pressure.
