As a Housing Resource Coordinator at Compass Health Network, you will play a pivotal role in the facilitation and enhancement of housing solutions tailored to meet the diverse needs of our client demographic and the broader community. Your primary focus will be on outreach and engagement with individuals facing housing instability or homelessness. You will also be tasked with cultivating relationships within the community, including current and prospective landlords, shelters, and various service providers.

Key Responsibilities:

  • Implement Housing First principles, harm reduction strategies, and low-barrier service approaches.
  • Participate in local Continuum of Care meetings to support coordinated entry and case management efforts.
  • Engage with individuals not currently utilizing CPR or CSTAR services who are in housing distress.
  • Facilitate referrals and provide transportation to initial appointments for services related to employment, behavioral health, substance use, and physical health.
  • Recruit landlords and establish strong relationships by providing information and making referrals to available units. Follow up with landlords to address tenant-related issues and maintain positive rapport.
  • Receive and act on referrals from Continuum of Care programs to assist individuals in securing housing.
  • Assist clients in navigating the SOAR process and ensure they receive the necessary support.
  • Support clients in their transition to permanent housing by evaluating their readiness to move on.
  • Coordinate efforts related to benefits acquisition, planning, and ongoing maintenance for clients.
  • Maintain an updated list of Medicaid re-certifications and monitor clients with inactive insurance.
  • Track outstanding Medicaid issues and liaise with relevant agencies to resolve them.
  • Provide reminders and assistance to staff as needed.
  • Collaborate with clients, Community Support Specialists, and agency contacts to complete re-certifications and manage public entitlements, including Social Security and Food Stamps.
  • Build relationships with Benefits Planning Specialists and Vocational Rehabilitation services to facilitate referrals and ensure continuity of care.
  • Deliver training to internal staff and external community partners on housing-related practices.
  • Offer Housing Support and skills training to agency housing initiatives.
  • Document all services provided to clients in the Electronic Health Record (EHR) system.
