Position Title: Case Manager - Support for Individuals Facing Homelessness

Role Overview:
As a Case Manager specializing in support for individuals experiencing homelessness, your primary responsibility will be to engage with underserved community members, including adults aged 18 and older, families, and youth transitioning out of foster care, who may be dealing with serious mental health issues, emotional disturbances, or co-occurring disorders. You will operate in various community settings where these individuals seek essential services.
Key Responsibilities:
- Serve as a primary responder to complex crises, addressing both psychological and environmental challenges.
- Adopt a holistic approach to ensure that care and discharge plans comprehensively address the physical, social, and emotional needs of clients.
- Assist clients in navigating housing resources and support systems.
- Conduct thorough assessments to identify client needs and facilitate access to necessary services.
- Maintain detailed documentation of services provided and develop individualized care plans with clients.
- Connect clients with community resources as required.
Qualifications:
- Bachelor's degree in health, psychology, counseling, social work, education, or a related behavioral health field.
- A minimum of one to two years of experience working with homeless populations or individuals with significant needs related to mental health or substance use disorders.
- Demonstrated knowledge, skills, and experience necessary for effective case management.
- Personal lived experience is preferred but not mandatory.
- Bilingual proficiency in Spanish is a plus but not required.
- A valid driver's license and eligibility for agency automobile insurance.
