Role Summary:

The Case Manager will deliver comprehensive case management services to adult clients receiving welfare across multiple counties. This role involves managing a designated caseload and creating tailored service plans that align with WFNJ and SAIF regulations.

Key Responsibilities:

  • Conduct regular evaluations and reassessments through consistent client interactions to assess their functioning and progress towards established goals. This includes but is not limited to: 1) Home visits; 2) Client telephone conferences; 3) Client office visits; 4) Visits to client work sites.
  • Communicate client progress effectively; provide crisis intervention and consultation to clients, their families, employers, and other stakeholders as required by the program; consult with supervisors regarding each situation prior to taking action; prepare client information for the Program Manager to assist in Monthly Service Report preparation.
  • Maintain detailed case files for each assigned client and connect clients to relevant supportive services, particularly for those eligible for Social Security disability benefits and/or medical deferment.
  • Advocate for clients and assist them with service applications when necessary.
  • Identify potential employers and create job opportunities tailored to the specific needs of clients, including facilitating placements in competitive work environments and nurturing relationships with current and prospective employers.
  • Participate in monthly case management meetings organized by BCBSS and/or other Regional County WFNJ collaborative meetings.
  • Assist the Program Manager with monthly presentations at relevant workforce centers as directed.
  • Communicate client progress, needs, and concerns through comprehensive case reviews and monthly reports.
  • Provide transportation for clients as needed.

Qualifications:

  • Bachelor's Degree in Social Work or a related field from an accredited institution, with experience in crisis intervention, counseling, mental health, substance abuse, domestic violence, family therapy, group work, and job development.
  • A valid driver's license is required, and candidates must be insurable under the agency's automobile insurance policy.
