Key Responsibilities
  • Assess and Address Client Needs: Conduct thorough assessments of client needs, develop and implement individualized treatment plans, and monitor progress.
  • Link Clients to Services: Connect clients with community resources, services, and entitlements to support their stabilization and re-housing goals.
  • Plan and Coordinate Services: Develop and implement service plans, coordinate with interdisciplinary teams, and arrange resources, including transportation and escort services.
  • Monitor and Evaluate Progress: Regularly review client progress, identify areas for improvement, and make adjustments to treatment plans as needed.
  • Communicate Effectively: Maintain open and transparent communication with clients, interdisciplinary teams, and stakeholders to ensure seamless service delivery.
Requirements
  • Education: High school diploma, HSE, or GED required.
  • Experience: Minimum of 2 years of social services experience, with a focus on working with homeless, mentally ill, substance abuse, and/or low-income populations.
  • Skills: Strong relationship-building skills, ability to work with diverse populations, and proficiency in Microsoft Office, including Word and Excel.
  • Preferred Qualifications: Experience working in a homeless shelter, knowledge of mental health, substance use, or homeless services, and bilingual (English/Spanish) skills.
Work Environment

This role operates in a shelter environment in Queens, NY, and requires the ability to work with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.

Physical Demands

This position requires regular standing, walking, sitting, stooping, climbing stairs, and/or kneeling, with occasional lifting and/or moving up to 10 pounds.
