Job Summary:
This role involves delivering in-home services and supportive interventions, which include case management, connecting clients to community resources, and coordinating treatment across agencies.
Conduct comprehensive assessments and evaluations of clients during intake, ongoing service delivery, and program termination.

Develop pertinent goals with measurable behavioral objectives to assess intervention progress and establish realistic timelines for achievement.

Document assessments, intervention plans, progress notes, and discharge summaries meticulously in client files, ensuring all entries are authenticated with appropriate dates and times.

Plan, communicate, and document aftercare strategies for clients completing the program or transitioning to other services within the continuum.

Collaborate effectively as part of an interdisciplinary team, which includes active participation in program and team meetings, providing support to colleagues, and engaging in performance improvement initiatives.

Attend weekly team case conferences and training sessions as required by Family Preservation Services.
Provide 24/7 coverage for assigned caseload.

What We Need to See:


A Bachelor's or Master's degree in social work or a related behavioral health field from an accredited institution.

Minimum of two (2) years of experience in counseling children and families.
Bilingual proficiency in Spanish is preferred.
A valid driver's license that meets the agency's insurance requirements.
