Case Manager

Lantern Community Services Inc is seeking an experienced Case Manager to join our team at Laurel Hall Shelter. As a Case Manager, you will be part of an interdisciplinary team aimed at stabilizing shelter clients by improving their independent living skills and assisting clients in moving toward their housing goals.
Key Responsibilities- Assess and Plan: Assess client needs, develop, monitor, and evaluate treatment plans and progress.
- Client Intake and Orientation: Review records and applications, conduct orientations, and determine clients' requirements by completing intake interviews.
- Treatment Programs: Establish treatment programs by setting schedules and routines, coordinating services, arranging resources, and providing personal support.
- Case Monitoring: Monitor cases by verifying clients' attendance, observing and evaluating treatments and responses, and advocating for needed services and entitlements.
- Communication and Collaboration: Communicate clients' progress by conducting weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to the therapeutic team and family, and identifying treatment influences.
- Education: High school diploma, HSE or GED, required.
- Experience: A minimum of 2 years of social services experience required, with experience working with the homeless, mentally ill, substance abuse, and/or low-income populations.
- Skills: Strong relationship-building skills, strong computer skills, including Word and Excel, and ability to work evenings and/or weekends on a rotating schedule.
- Preferred Qualifications: Preferred experience working in a homeless shelter, knowledge of mental health, substance use, or homeless services, and bilingual (English/Spanish) skills.
This job operates in a shelter environment in Queens, NY, and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.
Physical DemandsWhile performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally the employee will have to lift and/or move up to 10 pounds.
