About the Role

VNS Health is seeking a skilled Nurse Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality, cost-effective care to our patients.

Key Responsibilities
  • Coordinate and oversee the coordination of benefits and services for patients on your caseload.
  • Complete comprehensive care management and disease-specific assessments to identify patient needs and develop personalized care plans.
  • Maintain timely telephonic contact with patients to monitor their progress and address any concerns or issues that may arise.
  • Resolve complex issues and member complaints impacting the delivery of services, ensuring that patients receive the care and support they need to achieve optimal health outcomes.
  • Provide health education and support to patients and their caregivers, empowering them to take an active role in their healthcare.
  • Identify and address social determinants of health that may be impacting patient care, working to remove barriers and ensure that patients have access to the resources they need to thrive.
  • Collaborate with interdisciplinary teams to develop and implement care plans that are tailored to the unique needs of each patient.
  • Advocate for patients and their families, ensuring that their voices are heard and their needs are met.
  • Stay up-to-date with the latest clinical guidelines and best practices, applying this knowledge to inform your care management decisions.
Requirements
  • Current licensure and registration as a Registered Professional Nurse in New York State.
  • Certified Case Manager (CCM) certification within one year of employment.
  • Associate's or Bachelor's degree in nursing or a related field.
  • Minimum two years of experience in a healthcare-related field, with a focus on care management or managed care.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
