Position Overview:
As a Case Manager for Individuals Experiencing Homelessness, your role will involve engaging with underserved community members (individuals aged 18 and older, including youth transitioning out of care and families facing serious mental health challenges).
You will operate in various community settings where these individuals seek essential services, including shelters, health centers, and community programs.
Key Responsibilities:
Act as an initial point of contact, addressing complex situations that involve both psychological and environmental factors.
Adopt a holistic approach to ensure that care and discharge plans effectively address the physical, social, and emotional needs of clients.
Provide assistance with housing resources.
Conduct thorough assessments of clients' needs and identify available resources, including basic necessities, while facilitating access to these services.
Maintain comprehensive documentation of services rendered and develop individualized care plans with clients.
Connect clients to community resources as necessary.
Qualifications:
A Bachelor's degree in health, psychology, counseling, social work, education, or a related behavioral health field.
Minimum of one to two years of experience working with homeless populations or individuals with significant mental health needs.
Demonstrated knowledge, skills, and experience to effectively carry out case management duties.
Experience as a client with lived experience is preferred but not mandatory.
Bilingual proficiency in Spanish is preferred but not required.
A valid driver's license and eligibility for agency automobile insurance.
