Position Title: Case Manager - Supportive Assistance for Individuals and Families

Role Summary:

The Case Manager will deliver comprehensive case management services to adult clients receiving welfare across multiple counties. You will manage an assigned caseload and create tailored service plans that align with WFNJ and SAIF regulations.

Key Responsibilities:

  • Conduct ongoing assessments through regular interactions with clients to evaluate their functioning and progress towards established goals. This may include home visits, phone consultations, office meetings, and site visits.
  • Communicate client advancements; provide crisis intervention and guidance to clients, their families, employers, and other stakeholders as per program guidelines; consult with supervisors regarding each case before taking action; supply client information to the Program Manager for the Monthly Level of Service Report.
  • Maintain detailed case files for each assigned client and connect clients with suitable supportive services, particularly for those eligible for Social Security disability benefits and/or medical deferment.
  • Advocate for clients and assist them in applying for necessary services when appropriate.
  • Identify potential employers and create job opportunities tailored to the specific needs of clients, facilitating their placement in competitive work environments while nurturing relationships with current and prospective employers.
  • Participate in monthly case management meetings organized by BCBSS and/or other regional county WFNJ collaborative sessions.
  • Assist the Program Manager with monthly presentations at designated centers as directed.
  • Document and communicate client progress, needs, and concerns through case reviews and monthly reports.
  • Provide transportation for clients as necessary.

Qualifications:

  • Bachelor's Degree in Social Work or a related field from an accredited institution, with experience in crisis intervention, counseling, mental health, substance abuse, domestic violence, family therapy, group work, and job development.
  • A valid driver's license is required, and candidates must be insurable under the agency's automobile insurance policy.
