Our care team includes a variety of professionals such as physicians, advanced practice providers, pharmacists, care coaches, and behavioral health specialists, all working together to create a comprehensive care experience for seniors.

Patient Care Responsibilities:

  • Assess and manage patient care in alignment with established standards.
  • Monitor and evaluate the quality of care using available data and documentation.
  • Facilitate coordination of patient services, including specialty referrals and home health care.
  • Actively engage with the care team during daily meetings.
  • Collaborate with leadership to foster a cooperative practice environment.
  • Ensure timely and accurate medical record documentation.
  • Adhere to policies and protocols set by clinical leadership.
  • Participate in discussions regarding quality of care and outcomes.
  • Explore opportunities for service growth within the center.
  • Engage in the local primary care on-call program.
  • Maintain compliance with all licensing and accreditation requirements.
Utilize your expertise to make a meaningful impact.

Required Qualifications:

* Bachelor's degree

  • 8+ years of relevant experience
  • 2+ years in a leadership role
  • Active Nurse Practitioner/PA license in the relevant state
  • This position involves patient interaction and requires participation in a TB screening program.
  • A commitment to enhancing consumer experiences is essential.
Preferred Qualifications:

  • Master's degree in Nursing or Physician Assistant studies
Additional Information:

Scheduled Weekly Hours: 40

Compensation:
The compensation range reflects a good faith estimate for full-time employment. Actual pay may vary based on location and individual qualifications.

Pay Range: $102,200 - $140,700 per year

This position is eligible for a performance-based bonus incentive plan.
