Client Care Coordinator

Position Overview:
The Care Coordinator plays a vital role in conducting a thorough strengths-based evaluation of individuals, collaborating closely with team members to formulate a comprehensive care strategy. This role involves overseeing the execution of the care plan, identifying service providers or community resources, and facilitating regular team meetings while ensuring consistent home visits.
Key Responsibilities:
- Ensure the delivery of care aligns with strength-based, family-centered, and culturally sensitive principles.
- Provide guidance and training to service providers regarding the model's values.
- Monitor progress towards treatment objectives and collect necessary data for evaluation purposes.
- Manage a caseload within established financial guidelines, utilizing available resources to tailor services to the unique needs of children and families.
Qualifications:
- A minimum of three years of experience in community-based services for children and youth, or equivalent experience with relevant educational credentials.
- Preferred qualifications include a bachelor's degree in social work or a related field, along with experience in partnering with families in social services or educational settings.
- Strong communication skills and a commitment to empowering individuals with mental health challenges.
Additional Information:
- Bilingual skills, particularly in Spanish, are advantageous.
- Candidates must obtain CANS certification within 45 days of employment.
- A valid driver's license and auto insurance are required.

- Competitive hourly rate ranging from $24.03 to $25.48 based on educational qualifications.
