The Care Coordinator is tasked with conducting a thorough strengths-based evaluation of individuals, collaborating closely with team members to formulate a comprehensive care strategy. This role involves overseeing the execution of the care plan, identifying service providers or community resources, facilitating regular team meetings, conducting home visits, and monitoring all authorized services for client care. The Care Coordinator ensures that care is delivered in alignment with strength-based, family-centered, and culturally competent principles. Additionally, this position offers guidance and education to all service providers regarding these values, tracks progress towards treatment objectives, and ensures that all necessary data for evaluation is collected and documented. The incumbent will actively support and promote a commitment to racial and ethnic equity and antiracism.

Key Responsibilities

  • Manages a caseload within the financial guidelines of established protocols.
  • Utilizes available resources and flexible funding to ensure services are tailored to the specific needs of children and families.
  • Employs referral information, Child and Adolescent Needs and Strengths (CANS) assessments, and other relevant data to complete a strengths-based evaluation for use by the child and family team in collaboratively developing a care plan with clearly defined objectives.
  • Readily undertakes additional responsibilities as assigned to further the mission of CHOICES Inc.
Qualifications
  • Experience in providing community-based services to children, youth, and their families or caregivers in areas such as behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or related public sector human services for:
    • Three years with a high school diploma or equivalent; or
    • Two years with an associate's or bachelor's degree; or
    • One year with a master's degree or higher.
  • A minimum of a bachelor's degree in social work or a related human services field is preferred.
  • Two years of experience collaborating with youth and/or families in social services or educational contexts is preferred.
  • Experience in developing and managing individual service delivery budgets is advantageous.
  • Strong understanding and respect for youth and adults with mental health challenges, along with a commitment to empowering their families.
  • Excellent communication and writing skills; bilingual abilities (especially in Spanish) are a plus.
  • Must obtain CANS certification within 45 days of hire and maintain it thereafter.
  • Highly organized and detail-oriented.
  • Possession of a valid driver's license and auto insurance is required.
  • Demonstrated ability to:
    • Collaborate effectively with internal and external stakeholders, including community professionals.
    • Function effectively as part of a team.
    • Communicate effectively with various audiences through in-person interactions and electronic media.
    • Manage time and workload with minimal supervision.
    • Handle multiple priorities simultaneously.
Compensation: $24.03 per hour (bachelor's degree / $50,000 annually) or $25.48 per hour (master's degree / $53,000 annually)
