The Care Coordinator is committed to delivering services that reflect strength-based, family-centered, and culturally sensitive principles. Key Responsibilities:

  • Oversees a designated caseload while adhering to established financial guidelines.
  • Utilizes available resources and flexible funding to ensure services meet the specific needs of children and families.
  • Employs referral data, Child and Adolescent Needs and Strengths (CANS) assessments, and other relevant information to conduct a strengths-based evaluation for the child and family team to collaboratively establish a care plan with clear objectives.
  • Readily accepts additional responsibilities as assigned to further the mission of CHOICES Inc.
Qualifications:
  • Experience in providing community-based services to children, youth, and their families in areas such as behavioral health, child welfare, developmental disabilities, or related public service sectors for:
    • Three years with a high school diploma or equivalent; or
    • Two years with an associate's or bachelor's degree; or
    • One year with a master's degree or higher.
  • A bachelor's degree in social work or a related field is preferred.
  • Two years of experience working with youth and families in social services or educational settings is advantageous.
  • Experience in developing and managing individual service delivery budgets is beneficial.
  • Possesses a strong understanding and respect for individuals with mental health challenges and a commitment to empowering their families.
  • Excellent communication and writing skills are required. Bilingual abilities, particularly in Spanish, are a plus.
  • Must obtain CANS certification within 45 days of hire and maintain it thereafter.
  • Highly organized with a keen attention to detail.
  • A valid driver's license and auto insurance are mandatory.
  • Demonstrated ability to:
    • Collaborate effectively with internal and external stakeholders, including community professionals.
    • Function effectively as part of a team.
    • Communicate effectively with diverse audiences in person and through various electronic platforms.
    • Manage time efficiently and work independently with minimal supervision.
    • Handle multiple priorities simultaneously.
Compensation: Hourly Rate: $24.03 (bachelor's degree/$50,000 annually) or $25.48 (master's degree/$53,000 annually)
