Coordinated Entry and Intake Specialist

Families In Transition is seeking a highly motivated and compassionate Coordinated Entry and Intake Specialist to join our team. In this role, you will provide coordinated entry services across the greater Manchester area, conducting specialized intake assessments, eligibility determinations, and case management for subsidized housing programs.

Key Responsibilities:

  • Assess the needs of families seeking housing and determine the best resources for their situation
  • Maintain the waiting list for Coordinated Entry and ensure proper documentation is in place
  • Field and record pre-screening questions to determine household eligibility for housing crisis response services
  • Make effective referrals to local service and case management providers
  • Represent Families In Transition and the Coordinated Entry program in the community with a professional demeanor

Requirements:

  • High school diploma or GED
  • 2 years' experience in social services
  • Excellent interpersonal and communication skills
  • Ability to maintain accurate and efficient records and files
