Care Coordination Specialist Opportunity

Optum Home and Community Care is seeking a skilled Nurse Practitioner to join our Senior Community Care team. As a Care Coordination Specialist, you will play a critical role in delivering high-quality, patient-centered care to our members.

Key Responsibilities:

  • Provide comprehensive assessments and develop personalized care plans for members
  • Coordinate care with healthcare providers, family members, and caregivers to ensure seamless transitions and optimal outcomes
  • Monitor member needs and adjust care plans as necessary to ensure cost-effective, quality care
  • Collaborate with internal stakeholders to promote the CCM program and drive membership growth

Requirements:

  • Certified Nurse Practitioner through a national board
  • Graduate of an accredited Master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
  • Active and unrestricted license in the State of New Hampshire
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
