ROLE SUMMARY:

The Social Work department at Meals on Wheels San Francisco plays a crucial role in supporting clients to live safely and healthily in their residences. The Social Worker is tasked with performing initial and ongoing assessments for homebound adults with disabilities, facilitating referrals to community resources, and collaborating with meal delivery staff to ensure seamless service.

KEY QUALIFICATIONS:

  • A minimum of two years of direct experience working with seniors and a strong understanding of resources available for adults with disabilities in the area is highly preferred.
  • A degree in a relevant social service field may be considered alongside applicable work experience.
  • A Bachelor's degree in Social Work is preferred.
  • Ability to engage effectively with a diverse population.
  • Experience in working with seniors and/or adults with disabilities is essential.
  • Strong teamwork skills are necessary.
  • Exceptional organizational, communication, and documentation skills are required.
  • Proficiency in computer applications, including Microsoft Office, and familiarity with databases.
  • Bilingual abilities are advantageous, with a preference for Spanish, Cantonese, Russian, or Mandarin.
  • A valid driver's license, proof of insurance, and access to a vehicle are mandatory.
  • A background check, including a DMV record, is part of the application process.

RESPONSIBILITIES:

  • Conducting comprehensive in-home assessments on a weekly basis and annual evaluations for clients enrolled in the home-delivered meal program to ascertain eligibility and social service needs.
  • Maintaining confidential electronic records for each client served, ensuring documentation is accurate and timely.
  • Establishing and managing appropriate documentation, preparing reports, and entering client information into necessary databases.
  • Making suitable referrals to community resources and coordinating services to enhance client care.
  • Assisting in the development and updating of information regarding community resources for seniors.
  • Staying informed about and adhering to all agency policies and procedures.
  • Addressing client inquiries and managing situations as they arise.
  • Collaborating with meal delivery staff and other departments to ensure effective service delivery to clients.
  • Meeting with drivers to monitor client eligibility and safety, assisting with quarterly reassessments, and addressing any concerns.
  • Covering shifts as the Social Worker of the Day and managing requests during assigned times.
  • Providing limited support services to seniors requiring critical assistance, including care plan development and ongoing case monitoring.
  • Referring clients to volunteer services and addressing any related concerns.
  • Assisting with orientations for new volunteers as needed.
  • Participating in departmental meetings and professional development opportunities.
  • Attending relevant community meetings and presenting social work information as required.
  • Providing data entry support within the Social Work Department as necessary.
  • Participating in agency events as requested.
  • Submitting monthly reports to the Social Work Supervisor.
  • Delivering meals in emergency situations when necessary.
  • Performing additional duties as assigned.
