ROLE SUMMARY:
The Social Work department at Meals on Wheels San Francisco plays a crucial role in supporting clients to remain healthy and secure in their residences. The Social Worker is tasked with performing initial and ongoing assessments for homebound adults with disabilities, facilitating appropriate referrals to community resources, staying informed about available services, collaborating with meal delivery staff to ensure seamless service, addressing client needs as they arise, and offering support to a manageable caseload.
QUALIFICATIONS:
- Minimum of two years of direct experience working with seniors and knowledge of resources for adults with disabilities in the San Francisco area is highly preferred.
- A degree in a relevant social service field may be considered in conjunction with applicable work experience.
- A Bachelor’s degree in Social Work is preferred.
- Ability to engage effectively with a diverse population.
- Experience in working with seniors and/or adults with disabilities is essential.
- Strong teamwork skills are necessary.
- Excellent organizational, communication, and documentation skills are required.
- Proficiency in computer applications, including Microsoft Office, and familiarity with databases is expected.
- Bilingual candidates are encouraged to apply, especially those fluent in Spanish, Cantonese, Russian, or Mandarin.
- A valid driver's license, proof of insurance, and access to a vehicle are mandatory.
- A background check, including a DMV report, will be part of the hiring process.
RESPONSIBILITIES:
- Conducting in-home assessments on a weekly basis and annual evaluations for clients enrolled in the home-delivered meal program to assess eligibility, meal type, and additional social service needs.
- Maintaining confidential electronic records for each client served, ensuring documentation is accurate and timely.
- Establishing and managing appropriate records, preparing reports, and entering client data into relevant databases as required.
- Making suitable referrals to community resources and coordinating services to enhance client care.
- Assisting in the development and updating of information regarding community resources available for seniors.
- Staying informed and adhering to all agency policies and procedures.
- Responding to client inquiries and addressing situations as they occur.
- Collaborating with meal delivery staff and other departments to ensure effective service delivery to clients.
- Meeting with drivers to monitor client eligibility and safety, assist with quarterly reassessments, and address any concerns.
- Providing coverage for Social Worker of the Day shifts as assigned and managing all requests during these shifts.
- Offering limited support services to seniors requiring critical assistance, including care plan development and ongoing case monitoring.
- Referring clients to volunteer services and addressing any related concerns.
- Assisting with volunteer orientations for new client volunteers as needed.
- Participating in departmental meetings and professional development opportunities.
- Attending relevant community meetings focused on senior and social services as assigned.
- Presenting social work information at various meetings and workshops as requested.
- Providing data entry support within the Social Work Department as needed.
- Participating in agency events as requested.
- Submitting monthly reports to the Social Work Supervisor.
- Delivering meals in emergency situations when necessary.
- Performing additional duties as assigned.
COMPENSATION AND HOURS: The position offers a competitive hourly wage. This is a full-time role, requiring 40 hours per week, with occasional holiday work. Meals on Wheels provides a comprehensive benefits package, including paid sick leave, vacation, holidays, personal days, and a robust medical and dental plan.
