This is a dynamic opportunity to join LCMC Health as a Care Coordination Specialist, where you will play a vital role in ensuring seamless patient care transitions and efficient utilization management processes.
Key Responsibilities- Discharge Planning: Collaborate with the care management team to facilitate timely patient discharges, researching and coordinating post-acute care arrangements with preferred providers.
- Utilization Management: Assist in the transmission of reviews to payers, communicate pertinent information, and support concurrent insurance denials and appeals processes.
- Department Support: Provide clerical assistance to the department leadership, updating payer contact information and maintaining accurate vendor data.
- A combination of education and work experience, with a high school diploma or equivalent and 2 years of office/clerical experience in a healthcare setting.
- 4 years of experience in lieu of education.
