As a Nurse Practitioner (NP) at CenterWell Senior Primary Care, you will play a vital role in our collaborative healthcare environment. We prioritize value-based care, ensuring high-quality service for our patients.
Our multidisciplinary care team includes physicians, advanced practice professionals, pharmacists, care coaches, medical assistants, behavioral health specialists, quality coders, referral coordinators, and more. This diverse team approach allows us to deliver exceptional experiences for seniors.
Patient Care Responsibilities:
- Assess and manage patient care in line with established standards.
- Monitor patient care quality through data analysis and chart reviews.
- Coordinate patient services, including specialty referrals and home health care.
- Engage actively with the care team during daily meetings.
- Collaborate with the Regional Medical Director, physicians, and center administrators to foster a cooperative practice environment.
- Ensure timely and accurate medical record documentation, working closely with quality coders.
- Adhere to policies and protocols set by Clinical Leadership.
- Participate in discussions regarding care quality and outcome data.
- Explore growth opportunities for new or existing services within the center.
- Be part of the local primary care on-call program.
- Maintain compliance with all licensing and accreditation requirements.
Required Qualifications:
- 2 to 5 years of relevant experience preferred.
- Completion of an accredited Nurse Practitioner program with Board Certification.
- Active, unrestricted Nurse Practitioner license in the state of practice.
- Strong communication skills, both written and verbal, with a commitment to integrated team-based care.
- Willingness to adapt to a value-based care model.
- Excellent patient service skills.
- Basic computer proficiency, including email and electronic medical records.
- Certification to prescribe medications under a collaborating physician.
- Participation in the Tuberculosis (TB) screening program is required.
Preferred Qualifications:
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Experience managing a Medicare Advantage patient panel with a focus on coordinated care.
- Knowledge of Medicare guidelines and HEDIS quality indicators.
- Bilingual skills are advantageous.
