DEPARTMENT OVERVIEW: The Los Angeles LGBT Center's Health Services Department is a leading provider of specialized primary care and related services for the LGBTQ community. Our mission is to empower LGBTQ individuals to achieve healthier and more fulfilling lives through a wide range of client-focused services.
ROLE SUMMARY: The Clinical Social Worker will deliver psychosocial care management services and cultivate resources for clients under case management. Collaborating closely with the HIV Medical Care Coordination team, the role emphasizes the execution and monitoring of psychosocial care plans to ensure a holistic and integrated approach to client wellness.
KEY RESPONSIBILITIES:
- Collaborate within a multidisciplinary care team
- Conduct thorough psychosocial evaluations
- Provide brief interventions for mental health support
- Manage all psychosocial elements of client treatment
- Offer expert consultation to multidisciplinary treatment teams
- Develop and support the implementation of comprehensive care plans
- Assess client needs, strengths, and readiness for change
- Facilitate connections to medical, mental health, and substance abuse services
- Continuously evaluate client eligibility for public health programs
- Act as a liaison between clients and available services
- Organize necessary documentation for assistance programs
- Address transportation needs for clients
- Provide referrals and connections to partner services
- Maintain accurate and complete records of all services rendered
- Engage professionally with staff and clients
- Participate in team meetings and adhere to regulations
QUALIFICATIONS AND EXPERIENCE:
- At least 1 year of experience providing direct services to individuals living with HIV/AIDS
- Master's degree in Social Work
- Current AHA CPR certification
- Understanding of LGBTQ community issues
- Commitment to the organization's mission
- Excellent communication and organizational abilities
- Self-driven and dedicated
- Capacity to work independently while maintaining confidentiality
- Bilingual skills are advantageous
- Ability to engage effectively with diverse populations
