Key Responsibilities:
- Collaborate within a multidisciplinary care team to enhance client outcomes.
- Conduct thorough psychosocial evaluations to understand client needs.
- Implement brief mental health interventions as necessary.
- Manage all psychosocial dimensions of client treatment plans.
- Provide expert consultation to multidisciplinary treatment teams.
- Formulate and assist in executing comprehensive care strategies.
- Assess patient requirements, strengths, and readiness for change.
- Facilitate connections to medical, mental health, and substance abuse services.
- Continuously evaluate client eligibility for public health initiatives.
- Act as a bridge between clients and available services.
- Coordinate documentation for public and private assistance programs.
- Address transportation needs for clients effectively.
- Link clients to partner services for enhanced support.
- Maintain meticulous records of all services rendered.
- Engage professionally with both staff and clients.
- Participate in departmental meetings and adhere to regulatory standards.
- At least one year of experience in providing direct services to individuals living with HIV/AIDS.
- Master's degree in Social Work is required.
- Current CPR certification from AHA.
- Familiarity with issues affecting the LGBTQ community.
- Strong commitment to the organization's mission.
- Excellent communication and organizational abilities.
- Self-driven and dedicated to client confidentiality.
- Bilingual capabilities are advantageous.
- Proficient in working with diverse populations.
