Job Title: Clinical Nurse Care Manager

The Clinical Nurse Care Manager (CNCM) plays a pivotal role in orchestrating patient and family-centered healthcare initiatives. Utilizing advanced analytical skills, the CNCM is tasked with formulating, executing, and assessing personalized care strategies aimed at empowering patients and enhancing health outcomes. This position involves the coordination of resources and healthcare activities to facilitate effective transition management, ensuring that the needs of patients and their families are met through tailored follow-up and service coordination across various healthcare settings.

This role adheres to the standards set forth by the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses with Interpretive Statements, and the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, as well as the AAACN Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management.

The Commonwealth of Virginia is seeking a Clinical Nurse Care Manager to join a multidisciplinary team that includes physicians, nurse practitioners, licensed clinical social workers, psychologists, clinical research coordinators, and educational consultants. This collaborative team is dedicated to delivering exceptional care to patients with complex health needs and their families. Key responsibilities encompass scheduling outpatient appointments, managing hospital admissions, coordinating procedures and imaging studies, obtaining prior authorizations, and liaising with home health service providers.

Effective care coordination and transition management require professional evaluation, identification of patient risks, and stratification, as well as recognizing individual patient needs and preferences, including:

  • Exemplifying the use of the Professional Practice Model through nursing excellence, quality achievements, continuous learning, empowered leadership, innovation, and compassionate care.
  • Strategically planning, coordinating, and prioritizing patient care activities while considering the unique needs and desired outcomes of patients in collaboration with the inter-professional team.
  • Ensuring safety and continuity of care through meticulous documentation and effective hand-off processes.
  • Engaging and advising patients, families, and caregivers in healthcare decision-making.
  • Delivering health education and coaching tailored to specific issues within treatment and service plans.
  • Promoting health equity and enhancing population health through leadership initiatives.
  • Applying research and evidence-based practices to elevate patient care standards.
  • Guaranteeing continuity and consistency of care throughout the healthcare continuum.
  • Educating patients and families dealing with chronic illnesses about evidence-based practices.
  • Identifying support requirements, developing actionable plans, and initiating patient-centered care planning.
  • Contributing to problem-solving efforts and evaluating treatment outcomes.
  • Assisting with medication management.
  • Performing additional duties as assigned.

Minimum Qualifications:

  • Education: Bachelor of Science in Nursing from an accredited institution.
  • Experience: A minimum of 1 year of registered nursing experience is required, with 3 years of relevant experience preferred.
  • Specialty Board Certification is strongly preferred.
  • License/Certification: Must be licensed to practice as a Registered Nurse in the Commonwealth of Virginia. Current American Heart Association (AHA) Health Care Provider BLS certification is required.

Physical Requirements:

This position requires prolonged standing, frequent travel, and the ability to bend and stoop. Proficient communication, auditory, and visual skills are essential; attention to detail and legible writing are necessary. The role may involve lifting, pushing, or pulling weight, and exposure to chemicals, blood/body fluids, and infectious diseases may occur.
