POSITION TITLE: Senior Case Manager / Case Management Supervisor

DIVISION: Services

REPORTS TO: Shelter Director

LOCATION: Bronxwood, NY

SALARY: $48,000 - $52,000

POSITION SUMMARY:

The Senior Case Manager will oversee the daily operations of the case management team within a men's homeless shelter. This role requires collaboration with various on-site departments to implement effective problem-solving strategies and align operational goals. Proof of COVID-19 vaccination is mandatory.

KEY RESPONSIBILITIES:

  • Conducts intake processes and formulates comprehensive assessments for all new clients.
  • Establishes systems to ensure the quality and efficiency of case management services.
  • Ensures staff accurately identifies client needs, provides suitable referrals, and follows up on resources to support the achievement of service plan objectives.
  • Maintains proper documentation of assessments, service plans, and client interactions.
  • Reviews all necessary case management documentation and data collection tools.
  • Supervises service staff effectively.
  • Delivers clear, concise, and professional interventions in line with agency policies and evidence-based practices.
  • Fosters collaboration with community service providers.
  • Keeps all required documentation and reports up to date and submits them promptly.
  • Provides crisis intervention and stabilization support as needed.
  • Identifies program needs and offers recommendations.
  • Performs additional tasks as assigned.

QUALIFICATIONS:

Must possess an LMSW with a minimum of three years of relevant experience, including supervisory experience. Fluency in Spanish is highly desirable. Flexibility to work some evenings and occasional weekends is required. Strong writing skills, along with experience in assessment, direct practice, counseling, supervision, community outreach, group leadership, and program development, are preferred. Advanced proficiency in MS Word, Excel, and database management is essential. A valid NYS driver's license and personal vehicle are required.
