Position Summary:

The Senior Case Manager is essential in directing the overall operations of our program, working in close partnership with the Case Manager Supervisor. This role encompasses the planning, development, execution, monitoring, assessment, and ongoing improvement of processes. The Senior Case Manager will provide daily oversight and mentorship to the Housing Specialist, Employment Specialist, and Intake Specialist, ensuring effective collaboration under the supervision of the Case Manager Supervisor.

Key Responsibilities:

The Senior Case Manager will be tasked with, but not limited to:

  • Ensuring compliance with organizational policies and procedures, with an emphasis on effective conflict resolution.
  • Accompanying clients to housing viewings, HRA appointments, and facilitating check pickups from relevant agencies.
  • Collaborating with the Housing Specialist to prepare and submit housing documentation and applications.
  • Actively participating in the development of departmental goals, objectives, policies, and procedures, providing valuable recommendations for enhancements.
  • Assisting the Case Manager Supervisor in guiding a team of case managers in creating independent living plans, discharge plans, and post-care programs.
  • Providing direct supervision, monitoring, and evaluation of team members, ensuring effective execution of daily, weekly, and periodic tasks.
  • Delivering crisis intervention and emergency services as necessary.
  • Overseeing case managers to guarantee the provision or arrangement of all necessary and appropriate services, including housing, employment, and therapeutic placements, validated through thorough chart audits.
  • Conducting monthly chart audits and diligently following up to ensure adherence to relevant regulations.
  • Maintaining accurate and organized supervisory case notes, referrals, and related documentation.
  • Addressing facility-related issues diplomatically and facilitating conflict resolution.
  • Generating timely and comprehensive progress reports on a weekly, monthly, quarterly, and annual basis.
  • Collaborating with external agencies and individuals regarding resident records, rights, and responsibilities.
  • Planning and executing community outreach initiatives and related activities.
  • Acting as a liaison and representative for engagement with community organizations, as required.
  • Reviewing and assessing initial intake and evaluations for clients.
  • Managing a caseload of clients based on the program's census.
  • Reviewing and approving work and dorm passes for clients.
  • Contributing actively to case conferencing and client review processes.
  • Providing ongoing case management and counseling, extending support up to one year post-client departure.

Qualifications:

  • Bachelor's degree in social work, psychology, sociology, or a related field with a minimum of 2 years of direct social service experience.
  • Associate degree in human services or a related field with a minimum of 4 years of direct social service experience.
  • High School Diploma/General Equivalent Diploma with 8 or more years of direct social service experience.
  • Strong understanding of the challenges faced by homeless and special needs populations.
  • Expertise in case work and case management methodologies.
  • Demonstrated commitment to the mission of the organization.
  • Proven ability to collaborate effectively with diverse groups.
  • Strong organizational skills, meticulous attention to detail, and operational efficiency.
  • Exemplification of maturity, integrity, and sound judgment.

Physical Requirements and Work Environment:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 5 pounds occasionally.
  • Must be able to access and navigate various departments within the organization.
  • Ability to climb stairs.
