SALARY: $48,000 - $52,000
SUMMARY:
The Case Management Supervisor/Senior Case Manager is responsible for overseeing the daily operations of the case management team within a men's homeless shelter. This role requires collaboration with various on-site departments to enhance problem-solving strategies and ensure alignment in service delivery. Proof of COVID-19 vaccination is mandatory.
KEY RESPONSIBILITIES:
- Conducts intake processes and formulates comprehensive assessments for all new clients.
- Implements systems to ensure the quality and efficiency of case management services.
- Guides staff in identifying client needs, providing suitable referrals, and ensuring follow-up on resources to support clients in achieving their service plan objectives.
- Maintains accurate documentation of assessments, service plans, and client interactions.
- Reviews all necessary case management documentation and any utilized data collection tools.
- Supervises service staff effectively.
- Ensures interventions are clear, professional, and consistent with agency policies and evidence-based practices.
- Fosters collaboration with community service providers.
- Keeps all required documentation and reports organized and submitted punctually.
- Provides crisis intervention and stabilization support as necessary.
- Identifies program needs and offers recommendations for improvement.
- Performs additional related tasks as assigned.
QUALIFICATIONS:
Applicants must possess an LMSW with a minimum of three years of relevant experience, including supervisory experience. Proficiency in Spanish is highly desirable. Candidates should be available to work some evenings and occasional weekends as required. Strong writing skills, assessment capabilities, direct practice experience, counseling, supervision, community outreach, group leadership, and program development experience are preferred. Advanced knowledge of MS Word, Excel, and database management is essential. A valid NYS driver's license and personal vehicle are required.
