Position Summary:

The Senior Case Manager is integral to the effective management of our program's operations, working in close partnership with the Case Manager Supervisor. This role encompasses the planning, development, execution, assessment, and ongoing refinement of operational protocols. The Senior Case Manager will oversee and mentor the Housing Specialist, Employment Specialist, and Intake Specialist, ensuring effective collaboration under the direction of the Case Manager Supervisor.

Key Responsibilities:

The Senior Case Manager's duties include, but are not limited to:

  • Uphold strict compliance with organizational policies and procedures, emphasizing effective conflict resolution strategies.
  • Accompany clients to various appointments and assist with necessary logistics.
  • Work alongside the Housing Specialist to prepare and submit housing documentation and applications.
  • Actively contribute to the development of departmental goals, objectives, policies, and procedures, providing valuable insights for enhancements.
  • Support the Case Manager Supervisor in guiding a team of case managers in formulating independent living plans, discharge strategies, and post-care programs.
  • Deliver direct supervision, monitoring, and evaluation of team members to ensure the proficient execution of assigned tasks.
  • Provide crisis intervention and emergency support as required.
  • Ensure case managers facilitate or arrange all essential services, including housing, employment, and therapeutic placements, validated through thorough audits.
  • Conduct monthly audits and follow up to ensure adherence to relevant regulations.
  • Maintain organized and accurate supervisory case notes, referrals, and documentation.
  • Address facility-related issues diplomatically and facilitate conflict resolution.
  • Generate comprehensive progress reports on a regular basis.
  • Collaborate with external agencies regarding resident records, rights, and responsibilities.
  • Plan and implement community outreach initiatives and activities.
  • Serve as a liaison with community organizations as necessary.
  • Review and evaluate initial client intakes and assessments.
  • Manage a caseload of clients based on organizational needs.
  • Review and authorize client work and dorm passes.
  • Participate actively in case conferencing and client review sessions.
  • Provide ongoing case management and counseling, extending support for up to one year post-client departure.

Qualifications:

  • Bachelor's degree in social work, psychology, sociology, or a related field with a minimum of 2 years of direct experience in social services.
  • Associate degree in human services or a related field with at least 4 years of direct social service experience.
  • High School Diploma/General Equivalent Diploma with 8 or more years of direct social service experience.
  • Strong understanding of the challenges faced by homeless and special needs populations.
  • Expertise in casework and case management practices.
  • Demonstrated commitment to the mission of the Neighborhood Association.
  • Proven ability to work collaboratively with diverse groups.
  • Strong organizational skills, attention to detail, and operational efficiency.
  • Exhibit maturity, integrity, and sound judgment.

Physical Requirements and Work Environment:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to lift up to 5 pounds occasionally.
  • Capability to navigate various departments within the organization.
  • Ability to climb stairs as needed.
