A health system in New York is actively seeking a dedicated Health Home Supervisor to join their team. This role involves managing the Health Home Census, supervising case managers, and ensuring compliance with healthcare regulations.

Responsibilities:

  • Supervise and provide direction to 7 case managers, ensuring timely and accurate documentation.
  • Review and approve care plans, assessments, and documentation in compliance with Health Home regulations.
  • Collaborate with SBH Behavioral Health to identify and enroll appropriate patients for care management.
  • Coordinate care and services between Health Home, Behavioral, and Medical providers.
  • Provide crisis management and ensure proper incident documentation.
  • Train new employees and manage payroll, vacation, and elective days.
  • Participate in the Health Home Management Team and execute initiatives.

Qualifications:

  • Master's Degree in Social Work or related field.
  • 2 years of supervisory experience in a behavioral health setting.
  • 3 years of care management experience.

Desired Skills:

  • LMSW, LCSW, or LMHC preferred.
  • Proficiency in Microsoft Office, Excel, and EMR systems.
  • Bilingual in Spanish/English preferred.
