Key Responsibilities:
The role involves assisting patients in navigating post-acute care, with the ultimate aim of enhancing patient outcomes and satisfaction among referral sources. You will integrate evidence-based clinical guidelines, preventative measures, protocols, and various metrics to develop patient-centered transition plans that promote quality and efficiency in post-acute care delivery. Additionally, you will represent our regional branches in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and executive-level stakeholders.
Qualifications:
Essential:
- Graduation from an accredited nursing, therapy, or social work program.
- Current licensure in the state of practice.
- A minimum of two years of demonstrated field experience.
- Knowledge of federal, state, and local laws and regulations governing Medicare-certified home health and hospice operations.
- Basic technology skills, including proficiency with mobile devices.
Preferred:
- Registered nurse or physical therapist credentials.
- Three years of field experience.
- Previous experience in home health or healthcare sales.
Requirements:
- Valid state driver’s license.
- Automobile liability insurance as mandated by law.
- Reliable transportation in good working condition.
- Able to safely operate a vehicle in various weather conditions.
