Responsibilities:

Assist patients in navigating the post-acute care landscape, with the ultimate goal of enhancing patient outcomes and satisfaction among referral sources. Utilize evidence-based clinical guidelines, preventative measures, protocols, and other metrics to develop patient-centered transition plans that prioritize quality and efficiency in post-acute care delivery. Act as a representative for local branches in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and executive-level stakeholders.

Qualifications:

Essential Education and Experience:

  • Graduation from an accredited nursing, therapy, or social work program.
  • Current licensure in the state of practice.
  • A minimum of two years of relevant field experience.
  • Knowledge of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
  • Basic technology skills, including proficiency with mobile devices.

Preferred Education and Experience:

  • Registered nurse or physical therapist preferred.
  • Three years of field experience preferred.
  • Previous experience in home health or healthcare sales preferred.
Requirements:
  • Valid state driver’s license.
  • Automobile liability insurance as mandated by law.
  • Reliable transportation in good working condition.
  • Able to safely operate a vehicle in various weather conditions.
