Assist patients in navigating the post-acute care landscape, with the ultimate goal of enhancing patient outcomes and satisfaction among referral sources. Utilize evidence-based clinical guidelines, preventative measures, protocols, and other metrics to develop patient-centered transition plans that prioritize quality and efficiency in post-acute care delivery. Act as a representative for local branches in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and executive-level stakeholders.
Qualifications:Essential Education and Experience:
- Graduation from an accredited nursing, therapy, or social work program.
- Current licensure in the state of practice.
- A minimum of two years of relevant field experience.
- Knowledge of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
- Basic technology skills, including proficiency with mobile devices.
Preferred Education and Experience:
- Registered nurse or physical therapist preferred.
- Three years of field experience preferred.
- Previous experience in home health or healthcare sales preferred.
- Valid state driver’s license.
- Automobile liability insurance as mandated by law.
- Reliable transportation in good working condition.
- Able to safely operate a vehicle in various weather conditions.
