Position Overview:
The Clinical Pharmacist is a licensed professional responsible for delivering safe and effective pharmacy services to our patients, adhering to the policies and procedures of Option Care, as well as professional standards and regulatory requirements.
This entry-level role in clinical pharmacy within the alternate care setting involves coordinating patient services with the Nursing Department, monitoring patient progress, and educating patients, caregivers, and healthcare professionals on proper medication use.
Key Responsibilities:
- Assess the appropriateness of individual patients for home care services.
- Gather and organize patient-specific information to evaluate home care suitability.
- Design, recommend, monitor, and assess individualized pharmacy care plans for home care patients.
- Adjust pharmacy care plans based on monitoring data and relevant patient-specific information.
- Provide medication-use education to Option Care staff, healthcare professionals, home care patients, and their caregivers.
- Ensure seamless pharmaceutical care transitions between home and other patient-care environments.
- Oversee the use, maintenance, and troubleshooting of medication administration and related equipment for home care patients.
- Respond promptly and accurately to drug information inquiries from healthcare providers and home care patients.
- Manage investigational drug products in compliance with established regulations and company policies.
- Maintain confidentiality of patient and proprietary information.
Qualifications:
To qualify for this role, candidates must be graduates of an accredited School of Pharmacy and possess a current, active Pharmacy license from the State Board of Pharmacy.
Additionally, candidates should meet one of the following criteria:
- At least one year of experience in sterile IV admixture procedures.
- Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
- Completion of an ASHP accredited Pharmacy Residency Program.
- Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Equivalent experience as approved by the Vice President of Pharmacy or Senior Vice President of Clinical Services.
Basic qualifications include experience in customer service, proficiency in Microsoft Word, Excel, and PowerPoint, and a willingness to travel up to 5% for business purposes.
Preferred Qualifications:
Completion of an ASHP accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience is preferred, along with experience in home care, hospital, or clean room settings.
Compensation and Benefits:
Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. The pay range is competitive.
