Key Responsibilities
The primary duty of the Care Transition Coordinator is to ensure a smooth transition for patients moving from a healthcare facility to the care of an LHC Group hospice agency.
- Accountable for meeting admission goals as defined at the time of hire or during annual budget reviews.
- Assist the LHC Group agency in preparing for the acceptance of patients post-discharge from medical facilities.
- Support the Administrator in executing contracts for facility-based hospice services.
- Communicate hospice services and agency protocols to patients and their families.
- Engage family members and caregivers in the educational process, assessing their post-discharge learning needs.
- Participate in bi-weekly interdisciplinary group meetings to provide updates on accounts, customer requirements, and progress towards agency growth objectives.
- Minimum of one year of experience in hospice care or hospital case management.
- Current licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Social Worker (SW) in the state of practice.
- Strong organizational capabilities.
- Exceptional verbal and written communication skills.
- Comprehensive understanding of hospice eligibility criteria and coverage policies.
- Proficient in computer applications.
- Current CPR certification, valid driver's license, and access to reliable transportation.
