Bilingual Intake Coordinator

Program Description:Differential Response provides home visiting and case management service for families that have been reported to Child Protective Services and evaluated as low risk. Case managers help link families to resource centers, in-home therapy, and other services in schools and the community.
Job Summary:The Bilingual Intake Coordinator conducts daily intakes with HSA Social Worker, enters all pertinent referral information into the database, and coordinates with the Program Manager/Program Coordinator for case assignments. The Bilingual Intake Coordinator is responsible for conducting home visits and providing Differential Response services to clients, when necessary, in the San Mateo County. The incumbent will also conduct parent groups as required, assist in providing support to the Program Manager and Program Coordinator, and collaborate with other staff to ensure that the agency's missions are delivered.
Key Responsibilities:- Conduct daily intakes with HSA Social Worker and enter referral information into the database
- Coordinate with Program Manager/Program Coordinator for case assignments
- Conduct home visits and provide Differential Response services to clients, when necessary
- Conduct parent groups as required
- Assist in providing support to Program Manager and Program Coordinator
- Collaborate with other staff to ensure agency missions are delivered
- Weekly supervision with Program Manager/Program Coordinator
- Provide case management support to clients
- Facilitate and manage parenting groups
- Processing referrals
- Attending internal meetings, including training and other meetings as assigned by program manager
- Assist in providing support to ensure deadlines are met
- Coordinates with Program Coordinator to review and ensure appropriateness of client referrals; assign clients to case managers' caseloads
- Create MDT log with cases received and email to corresponding County staff
- Input of referral information (narrative and demographics) into ETO and touch points
- Ensure DR protocols and County monitoring systems are followed
- Scheduling of MDT meetings with HSA (as needed per request from HSA on after-care cases)
- Attend specific MDT meetings, as well as other county meetings assigned
- Responsibility for compliance with program guidelines
- Maintain confidential records and case notes with clients
- Ensure timely submission of monthly, quarterly, and annual data to Program Manager and Program Coordinator
- Maintain awareness of contract requirements and ensure that these are met
- Support case managers with referrals
- Tracking of resource line calls received and referrals given
- Cultivate and maintain strong, positive relationships with colleagues, other agency departments, and community agencies
- Ordering office supplies, arranging facility repairs as needed for all program staff
- Attend all staff meetings, mandatory training, and client activities as needed
- Other duties as assigned by the Program Coordinator, Program Manager, or Department Director
- Able to demonstrate coordination/management skills and experience in providing support to program activities
- Strong organization, communication, and leadership skills
- Ability to function as a member of a multidisciplinary team with diplomacy and professionalism
- Ability to work as a team member when necessary to accomplish tasks supporting the agency's mission
- Experience with home visiting programs and community-based work
- Understanding of diversity issues, willingness to explore self in relation to these issues, and ability to provide culturally informed services to diverse clientele
- Strong written and oral communication skills; excellent organizational skills
- Strong computer skills. Comfort with Word, Excel, PowerPoint, and client database
- Enjoys working with a large, dynamic team, supporting families with children
- Demonstrable ability in working with challenging clients
- Well organized, dependable, and punctual
- Ability to prioritize and manage multiple tasks to meet specific deadlines
- Flexibility in meeting and changing priorities as the work requires
- Able to function as part of a team and possess strong interpersonal skills
- Leadership skills and abilities
- Work as part of multidisciplinary team in an office environment
- Provide treatment services in homes and in office environments
- Travel to and from work sites
- Handle confidential information and documents, as well as potential crisis situations
- A valid CA Driver's License: his/her own car and valid car insurance is required
- HR clearance required, including TB test, and Department of Justice Live Scan Fingerprint clearance
- Demonstrable and verifiable skills, abilities, and qualities commensurate with specific position requirements
- Bachelor's degree in social work (or related field) or demonstrate equivalent knowledge/experience minimum of 4 years
