Job Summary:

As an Intake Coordinator at Self-Help for the Elderly, you will play a vital role in implementing the patient intake process, ensuring seamless transitions for our patients. Your primary responsibilities will include assessing patient needs, verifying insurance benefits, and coordinating clinical staff schedules. You will act as the point of contact for referrers, fostering positive relationships and ensuring timely assignments of clinical staff. Your exceptional communication and negotiation skills will be essential in engaging with patients, families, and clinical team members. Key Responsibilities:

  • Assess patient needs and qualifications, ensuring safe and effective care delivery.
  • Verify insurance benefits, including eligibility and service authorization procedures.
  • Coordinate clinical staff schedules, ensuring timely assignments and effective care delivery.
  • Act as the point of contact for referrers, fostering positive relationships and ensuring seamless transitions.
  • Collaborate with clinical team members, patients, and families to ensure comprehensive care delivery.
  • Maintain up-to-date knowledge of insurance companies and their practices.
  • Participate in quality assurance and performance improvement activities, contributing to the development of quality management reviews and evaluations.
  • Ensure compliance with regulations and office standards, maintaining accurate and timely documentation.

Requirements:

  • Current California Licensed Vocational Nurse (LVN) or equivalent.
  • Minimum one year of professional nursing experience in home health, hospice, acute, or sub-acute settings.
  • Preferred experience in healthcare operations and management.
  • Proven ability to lead program intake efforts and engage with diverse stakeholders.
  • Excellent communication, negotiation, and interpersonal skills.
  • Bilingual in English and Cantonese preferred.
  • Must pass health screening, physical examination, and background check.
  • Current CPR certification.
  • Ability to travel via public transportation or personal vehicle for job duties.
