Role Overview:
The Senior Manager of Transitions of Care (TOC) will play a pivotal role in our vision to establish a virtual "blue zone" for our members, promoting longer and healthier lives through timely and coordinated care.

In instances where the conventional healthcare system falls short, our members can depend on Devoted Medical, an innovative care delivery system dedicated to treating each member with familial care.

The TOC program is one of our largest clinical initiatives, focusing on ensuring that thousands of members transition safely back home after hospitalization.

From the moment of admission through 30 days post-discharge, our skilled virtual interdisciplinary teams provide personalized and comprehensive care to address clinical, social, and logistical challenges that may hinder post-discharge stability.

This member-focused approach has resulted in significant reductions in post-discharge complications and readmissions, and we are continually refining our care model to keep more members safe at home.

Your Responsibilities:
As a transformational clinical leader, you will:

  • Inspire and cultivate a collaborative, patient-centered team of nurses and care coordinators committed to enhancing post-acute patient experiences and outcomes.
  • Drive the hiring, training, and performance management strategies for frontline teams and managers.
  • Monitor and manage staff performance against key performance indicators (KPIs) to reduce avoidable admissions and readmissions.
  • Collaborate closely with clinical and operational leaders to shape the program's strategy and execution.

Qualifications:
We are seeking candidates with:

  • 5+ years of experience leading high-performing clinical teams in transitional care or care management.
  • 3+ years of experience in coaching and developing other leaders.
  • An active RN license in good standing, and/or a BSN or equivalent degree (master's preferred).
  • Experience in post-acute medicine, nursing, and case management processes.
  • A strong commitment to interdisciplinary care.

Compensation and Benefits:
The salary range for this position is $105,000 - $174,000 annually, with opportunities for growth within the role. Our total rewards package includes:
