Job Overview
We are seeking an experienced Service Coordinator - Case Manager III to join our team at Medical Temporaries, Inc. This is a full-time/temp position requiring availability between 8:30AM-5:00PM, Monday through Friday. The successful candidate will be responsible for providing service coordination/case management services to children with developmental delays and their families.

About the Role
This role requires a minimum of 2 years of experience working with the infant to 3-year-old ID population. The ideal candidate will have a valid driver's license, BLS certification, and flexible schedule. They must also be able to pass a background check and drug screen. In addition, they should possess excellent written and verbal communication skills, as well as the ability to multitask in a fast-paced environment.

Responsibilities
The successful candidate will be responsible for assessing, planning, linking, monitoring, and advocating for children and their families. They will also complete clinical documentation in accordance with agency and regulatory guidelines. Additionally, they may be required to provide services in the community or in the client's home, and may need to transport clients in a minivan or car.

Requirements
To be successful in this role, you will need:
  • Minimum of 2 years of experience working with the infant to 3-year-old ID population.
  • Valid driver's license and acceptable driving record according to the department of motor vehicles.
  • BLS certification.
  • Flexible schedule.
  • Ability to pass Background Check and Drug Screen.
  • Excellent written and verbal communication skills.
