Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Patient Care Coordinator. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support. This is a full-time position that requires excellent communication skills, a compassionate bedside manner, and the ability to work effectively in a fast-paced environment.

About the Role

The successful candidate will provide outpatient nursing care to patients, assist physicians with examinations and procedures, and perform basic patient care as ordered. They will also administer medications, observe patients for reactions, and respond to life-threatening situations based on nursing standards and protocols.

In addition to these responsibilities, the Patient Care Coordinator will maintain accurate medical records, prepare exam rooms, restock supplies, and sterilize equipment. They will also coordinate care with other healthcare providers, local pharmacists, and community services to ensure optimal patient outcomes.

Responsibilities

• Provide high-quality patient care, including triage, medication administration, and vital sign monitoring

• Assist physicians with examinations, minor surgeries, and diagnostic procedures

• Maintain accurate and detailed medical records, including charting and documentation

• Prepare exam rooms, restock supplies, and sterilize equipment

• Coordinate care with other healthcare providers, local pharmacists, and community services

• Respond to patient phone calls and messages in a timely and professional manner

Requirements

To be considered for this rewarding opportunity, you must possess a current Virginia LPN license, completion of BLS class, and re-certification every 2 years. You should also have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
