Job Summary:

We are seeking a skilled Clinical Liaison to facilitate Hospice and Palliative Care referrals from all sources. The ideal candidate will have a strong background in nursing and experience in hospice care. The successful candidate will be responsible for obtaining reviews and coordinating the necessary information to complete the clinical assessment of the patient for the correct program and location within that program.

Key Responsibilities:
  • Facilitate Hospice and Palliative Care referrals from all sources
  • Obtain reviews and coordinate necessary information for clinical assessment
  • Ensure accurate placement of patients in Hospice and Palliative Care programs

Requirements:
  • Associate Degree or Diploma in Nursing
  • New Mexico RN License required
  • 1 year of hospice experience required
  • 1 year of acute care experience strongly preferred
