As a key member of our healthcare team, you will be responsible for coordinating with interdisciplinary teams to ensure timely completion of MDS assessments in accordance with state and federal regulations.

Key Responsibilities:

  • Notify and coordinate with interdisciplinary teams for MDS assessment completion
  • Ensure compliance with state and federal regulations
  • Develop and maintain accurate and up-to-date patient records
  • Collaborate with healthcare professionals to ensure high-quality patient care

Requirements:

  • LVN license required
  • Minimum 1 year of LTC experience
  • Strong organizational and communication skills
  • Ability to work independently and as part of a team
