Position Overview:

The MDS Coordinator plays a pivotal role in overseeing the Resident Assessment Instrument (RAI) process and care planning within our licensed nursing facility. This position collaborates closely with the Director of Nursing to ensure the delivery of health-related services aligned with MDS and care planning functions.


Key Responsibilities:
  • Maintain up-to-date knowledge of Medicare and Medicaid provider guidelines.
  • Conduct pre-admission assessments for prospective residents.
  • Evaluate residents for Medicare payment eligibility, including clinical qualifications and Medicare day availability.
  • Lead an interdisciplinary team in completing comprehensive MDS assessments, including Care Area Assessments (CAA) and CAA Guidelines.
  • Ensure timely completion of RAI processes for all assessments, adhering to state and federal regulations.
  • Monitor resident health to identify significant changes and manage the re-assessment process.
  • Analyze MDS assessments prior to submission to the Center for Medicare/Medicaid Services (CMS) using standard organizational software.
  • Transmit RAI data to CMS on a scheduled basis and upload MDS data to the QAPI software routinely.
  • Collaborate with billing staff to ensure accuracy and timeliness in Medicare billing.
  • Guide the interdisciplinary team in developing individualized care plans based on assessments.
  • Provide education to team members and staff on documentation practices related to care plans.
  • Conduct regular audits of services provided by internal and external providers to ensure documentation accuracy.
  • Monitor Quality Measures Reports monthly, focusing on metrics that impact the Federal Five Star System.
  • Assist interdisciplinary teams in addressing MDS accuracy issues related to quality measures.
  • Engage in action planning with the interdisciplinary team to improve quality measures.
  • Coach the team to enhance processes for MDS accuracy.
  • Explain procedures and treatments to residents to foster understanding and cooperation.
  • Support the planning and organization of the Health Center to ensure competent nursing care is available at all times.
  • Interpret and apply established policies in accordance with nursing standards.
  • Participate in community quality assessment and improvement initiatives.
  • Adhere to all personnel policies, community procedures, and federal and state regulations, including HIPAA compliance.

Qualifications:
  • Bachelor's degree in Nursing.
  • Current Registered Nursing license in good standing.
  • Experience with the CMS Resident Assessment Instrument process.
  • Knowledge of OBRA/Federal/State regulations.
  • Preferred: Three years of MDS experience in a Long-Term Care environment.
  • Familiarity with standards of care and practices relevant to geriatric services and complex medical conditions.
