Responsibilities
  • Oversee the RAI Process.
  • Collaborate with the interdisciplinary team to assess patient/resident needs.
  • Actively participate in Care Coordination meetings.
  • Ensure timely and accurate MDS assessments in compliance with state and federal regulations.
  • Accurately code MDS assessments based on medical record reviews, observations, and interviews with staff, patients, residents, and families.
  • Maintain a tracking system for MDS assessment schedules, including due dates.
  • Coordinate Care Plan conferences with the interdisciplinary team, patients, residents, and families.
  • Review and maintain all State and Federal reports, making necessary corrections promptly.
  • Monitor Quality Measures to ensure MDS accuracy in supporting these measures.
  • Provide education regarding the RAI Process.
  • Coordinate and complete electronic submissions of required documentation to the State database and other entities in accordance with company guidelines and regulations.
  • Ensure compliance with Medicare and Medicaid regulatory guidelines.

Qualifications

Preferred: Two years of MDS experience, but not mandatory.

Must possess a current registered nursing license in the state of employment.

Experience in Long-Term Care is preferred.

Required: Proficiency in computer skills, including data entry and word processing.

Familiarity with the MDS 3.0 (current version of minimum data set) is essential.

Knowledge of nursing and medical practices, as well as relevant laws and regulations pertaining to skilled nursing facilities, is necessary.

Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures essential for delivering quality care.

Registered Nurse designation is required.
