Position Overview:

The MDS Coordinator plays a crucial role in overseeing the Resident Assessment Instrument (RAI) process and care planning within our licensed nursing center. This position supports the Director of Nursing in managing health-related services, specifically focusing on MDS and care plan functions.


Key Responsibilities:
  • Maintain up-to-date knowledge of Medicare/Medicaid provider guidelines.
  • Conduct pre-admission assessments for prospective residents.
  • Evaluate residents to determine eligibility for Medicare services, including clinical qualifications and Medicare day availability.
  • Lead an interdisciplinary team in completing comprehensive assessments, including Minimum Data Set (MDS) and Care Area Assessments (CAA).
  • Ensure timely completion of the RAI process for all assessments, adhering to state and federal regulations.
  • Monitor resident health status to identify significant changes and initiate re-assessment processes.
  • Analyze MDS assessments prior to submission to the Center for Medicare/Medicaid Services (CMS) using organizational software.
  • Transmit RAI data to CMS on a scheduled basis and upload MDS data to quality assurance software.
  • Collaborate with billing personnel to ensure accurate and timely Medicare billing.
  • Supervise the development of individualized care plans based on assessments.
  • Provide education to the interdisciplinary team and nursing staff on documentation standards.
  • Conduct audits of services provided to validate billing accuracy and compliance.
  • Monitor quality measures monthly, focusing on those affecting the Federal Five Star System.
  • Assist interdisciplinary teams in resolving MDS accuracy issues and participate in quality improvement initiatives.
  • Educate residents about procedures and treatments to foster understanding and cooperation.
  • Support the planning and organization of the Health Center to ensure continuous competent nursing care.
  • Adhere to established policies and regulations, including HIPAA privacy laws.

Qualifications:
  • Bachelor's degree in Nursing.
  • Current Registered Nursing license in good standing.
  • Experience with the CMS Resident Assessment Instrument process.
  • Knowledge of OBRA/Federal/State regulations.
  • Preferred: Three years of MDS experience in a Long-Term Care setting.
  • Familiarity with standards of care for geriatric services and complex medical conditions.
