Position Overview

The Case Manager plays a vital role within the patient care delivery team, demonstrating efficiency in a dynamic healthcare environment. Utilizing clinical expertise and independent judgment, the Case Manager engages in the nursing process, which includes assessment, care planning, intervention, and evaluation, all within a patient-centered care framework.

Key Responsibilities

  • Maintain comprehensive knowledge of specialized case and utilization management methodologies.
  • Ensure that medical appropriateness criteria are met throughout the patient's care journey, from admission to discharge.
  • Collaborate with interdisciplinary teams to implement individualized discharge plans that address the ongoing healthcare needs of patients.
  • Continuously reassess care plans to adapt to the evolving needs and conditions of patients.
  • Act as a change agent within the nursing profession, promoting positivity and embracing new knowledge and innovations.
  • Engage in committees and task forces to influence clinical practice and decision-making.

Qualifications

The ideal candidate will possess an Associate Degree in Nursing, with a commitment to further education in a Bachelor of Science Degree nursing program. A minimum of one year of patient care experience is required, alongside a California Registered Nurse (RN) license and Basic Life Support (BLS) certification. Preferred qualifications include two years of experience in Hospital Case Management or Utilization Management and nationally recognized certification in the field.
