Position Overview
The Case Manager plays a pivotal role within the patient care delivery framework, operating effectively in a dynamic healthcare environment. This role requires the application of clinical knowledge, sound judgment, and discretion in executing the nursing process, which encompasses assessment, care planning, intervention, and evaluation, all within a patient-centered care model.
Key Responsibilities
- Utilize specialized case management methodologies to ensure that medical appropriateness criteria are consistently met throughout the patient’s care journey.
- Collaborate with a multidisciplinary team to develop and implement a comprehensive and individualized discharge plan that addresses the ongoing healthcare needs of patients.
- Continuously reassess the plan of care to ensure it remains aligned with the evolving needs and conditions of patients.
- Act as a change agent within the clinical setting, promoting positivity and embracing new knowledge and innovations.
- Engage in decision-making processes related to clinical practice by participating in committees and staff meetings.
- Perform additional duties as required to support the overall mission of the organization.
Qualifications
Applicants should possess an Associate Degree in Nursing, with a commitment to enroll in a Bachelor of Science in Nursing program if hired. A Bachelor's Degree in Nursing is preferred. A minimum of one year of patient care experience is essential, along with at least two years of experience in Hospital Case Management or Utilization Management.
Licensure as a Registered Nurse in California is mandatory, along with Basic Life Support (BLS) certification from the American Heart Association. Preferred qualifications include nationally recognized certification in Utilization Management or Case Management.
Skills Required
- Proficient in English, with the ability to communicate effectively both verbally and in writing; proficiency in Spanish is an advantage.
- Familiarity with computer systems and software relevant to the position, including Microsoft Office Suite and electronic medical records.
- Ability to work independently and manage multiple tasks efficiently while maintaining attention to detail.
- Strong critical thinking and problem-solving skills, with the capacity to work under pressure.
