About the Role

SAC Health is seeking a skilled Licensed Vocational Nurse to join our team as a Patient Care Specialist. As a key member of our medical team, you will play a vital role in delivering exceptional patient care and contributing to our organization's mission to empower patients and their families to live vibrant and healthy lives.

Key Responsibilities
  • Manage Patient Flow and Relations: Coordinate patient care activities and ensure seamless communication with patients, families, and healthcare teams.
  • Provide Nursing Care: Utilize your knowledge of basic nursing principles, practices, and ethics to provide high-quality patient care, including administering medications, performing procedures, and documenting patient information.
  • Collaborate with Healthcare Teams: Work closely with clinicians, physicians, and other healthcare professionals to ensure comprehensive patient care and achieve optimal outcomes.
  • Assist with Back-Office Duties: Support the back-office team with tasks such as vital sign collection, injections, and specimen processing, ensuring accurate and timely completion of tasks.
  • Communicate Effectively: Develop strong relationships with patients, families, and healthcare teams through effective communication, active listening, and empathy.
  • Participate in Quality Improvement: Contribute to quality control functions, including monitoring expired medications, maintaining equipment, and participating in quality improvement initiatives.
  • Support Team Development: Assist with the orientation of new employees, participate in team huddles, and contribute to a positive and supportive team environment.
Requirements
  • Education: High School Diploma or equivalent required; Graduate of an accredited LVN program required.
  • Licensure/Certification: Current, active, and unrestricted LVN License required; current Basic Life Support (BLS) certification required (must be American Heart Association or Red Cross accredited program).
  • Experience: Minimum of one (1) year experience as an LVN preferred.
  • Technical/Motor Skills: Proficient computer skills, including data entry; ability to use office equipment, such as fax machines, scanners, and printers.
  • Interpersonal Skills: Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Mental Abilities: Prioritize tasks, follow instructions, and recognize and report safety problems or other problem situations.
