Position Overview

Summary:

The role involves delivering both direct and indirect nursing services to individual patients or groups.

Key Responsibilities:

1. Conducts comprehensive admission evaluations to establish a clear understanding of the patient's condition.

2. Maintains meticulous documentation that integrates the care plan, interventions, and outcomes, reflecting the patient's physical, psychological, and emotional health. Performs annual audits on documentation from three distinct patient charts.

3. Completes transfer and discharge paperwork, assessing the patient's requirements to determine the most suitable level of care.

4. Adheres to the five rights of medication administration, clearly communicating to patients and their families the medications being administered.

5. Evaluates the patient before medication administration, taking into account allergies, vital signs, potential side effects, and effectiveness, intervening as necessary.

6. Implements pain management strategies based on patient feedback and assessment findings, documenting responses in accordance with hospital protocols. Identifies high-risk medications and utilizes reference materials as required.

7. Evaluates patient safety risks upon admission, following hospital policies to ensure patient safety and initiating appropriate interventions.

8. Assesses the educational needs of patients and their families, planning and delivering tailored instruction using suitable teaching methods, and documenting responses to the education provided.

9. Applies the five rights of delegation, coordinating and overseeing nursing care delivery, while maintaining accountability for delegated tasks and clarifying the roles of team members.

10. Takes on the responsibilities of charge nurse, demonstrating independent problem-solving, decision-making, and organizational skills.

11. Proactively addresses unit staffing requirements in relation to patient acuity and census, assisting in the orientation and training of new nursing staff.

12. Monitors the patient's skin condition upon admission, during each shift, and at discharge, as well as during any changes in condition.

13. Initiates preventive measures for patients at risk of skin breakdown, staging pressure ulcers according to established guidelines and implementing protocols based on assessments.

14. Complies with Nursing Standards of Practice, including assessment, care planning, interventions, and evaluation of care plans.

15. Operates according to unit-specific standards, conducting walking rounds for shift reports and introductory rounds with assigned patients at the start of each shift.

16. Ensures compliance with all responsibilities associated with the position.

17. Performs additional duties as required.

Additional Information:

Must possess a valid RN license to practice in the state. Basic Cardiac Life Support (CPR) certification is required within three months of hire.

Certifications such as ACLS, PALS/ENPC, and trauma hours may be required based on unit standards.

Working Conditions:

Manual: Requires significant manual skills and dexterity.

Occupational: Involves considerable occupational risk.

Physical Effort: Requires very heavy physical effort, potentially exerting up to 50 lbs. of force.

Physical Environment: Significant exposure to various environmental factors.
