Position Overview
Summary:
The Clinical Nurse Specialist is responsible for delivering both direct and indirect nursing care to individual patients or groups of patients.
Key Responsibilities:
1. Patient Assessment:
- Conduct comprehensive admission assessments to establish a clear understanding of the patient's condition.
- Maintain detailed documentation that integrates the care plan, interventions, and outcomes, reflecting the patient's physical, mental, and emotional well-being.
- Perform ongoing reassessments on a shift-by-shift basis.
- Complete transfer and discharge documentation, assessing the patient's needs to determine the appropriate level of care.
2. Medication Administration:
- Adhere to the five rights of medication administration, clearly communicating to patients and their families about the medications being given.
- Evaluate the patient prior to medication administration, considering allergies, vital signs, potential side effects, and effectiveness, intervening as necessary.
- Manage pain based on patient feedback and assessment findings, documenting responses according to established standards.
- Identify high-risk medications and utilize appropriate references when necessary.
3. Patient Safety:
- Assess patients for potential safety risks upon admission.
- Follow hospital policies and procedures to ensure patient safety.
- Implement interventions based on identified risk factors.
4. Patient and Family Education:
- Evaluate the educational needs of patients and their families.
- Develop and deliver tailored educational content using suitable teaching methods.
- Assess and document the responses of patients and families to educational interventions.
5. Delegation:
- Apply the five rights of delegation effectively.
- Oversee and coordinate nursing care delivery, including the delegation of tasks.
- Maintain accountability for delegated care and clarify the roles of team members involved.
6. Leadership:
- Foster a positive work environment through exemplary behavior and teamwork.
- Assume the role of charge nurse, demonstrating independent problem-solving and organizational skills.
- Address unit staffing needs proactively in relation to patient acuity and census.
- Participate in the orientation and training of new nursing staff.
7. Care Delivery:
- Comply with Nursing Standards of Practice, including assessment, care planning, interventions, and evaluation of care plans.
- Practice in accordance with unit-specific standards.
8. Additional Duties:
- Perform other responsibilities as assigned.
- Ensure compliance with all responsibilities related to the position.
Qualifications:
Education: Associate's Degree in Nursing required.
Licensure:
Must possess a valid nursing license to practice.
Experience:
Minimum experience of zero to six months preferred.
Certifications:
Basic Cardiac Life Support (CPR) certification is required within three months of hire.
Working Conditions:
Manual dexterity and significant physical effort are required, with the ability to exert up to 50 lbs. of force. Exposure to various environmental factors is expected.
