Position Summary:

TEEMA Solutions is seeking a committed and empathetic Social Worker to deliver essential medically-related social services within a long-term care environment. This role is vital in ensuring the optimal mental and psycho-social well-being of residents in our skilled nursing facility, dedicated to serving the rehabilitative needs of Veterans.

As a Social Worker, you will be responsible for managing a personal caseload while ensuring adherence to all relevant federal and state regulations, as well as internal policies. Additionally, you will lead a team of social workers, providing necessary guidance and support to guarantee the effective execution of the social services program.

Key Responsibilities:
  • Program Oversight: Design, implement, and supervise the social services program, ensuring timely assessments and accurate documentation.
  • Team Leadership: Manage and mentor a team of social workers, conduct performance reviews, and oversee staffing decisions.
  • Discharge Coordination: Develop and communicate clinically appropriate discharge plans to all relevant parties.
  • Regulatory Compliance: Monitor Medicare notifications, ensure adherence to federal laws, and manage referrals to VA specialists.
  • Resident Advocacy: Collaborate with residents, families, and staff to uphold Resident Rights and manage grievance processes.
  • Financial Oversight: Assist in preparing departmental budget forecasts, monitor spending, and manage staff overtime.
  • Caseload Administration: Maintain a personal caseload of Veterans/Members, ensuring high-quality and compliant care.
  • Policy Formulation: Review and create policies to ensure regulatory compliance. Participate in meetings and special projects.
Required Qualifications:
  • Knowledge: Familiarity with Veterans Administration, CMS, and state regulatory standards.
  • Expertise: Strong understanding of social services programs, particularly in long-term care settings.
  • Skills: Ability to manage psychiatric conditions and geriatric health issues.
  • Competence: Proficient in care plan development, case management, and program planning.
  • Leadership: Strong leadership, supervisory, and communication abilities.
Education and Experience:
  • Experience: Minimum one year of supervisory experience in a healthcare environment, with direct involvement in long-term care.
  • Education: Master’s degree in Social Work (LMSW) required, with an active social work license from the Arizona Board of Behavioral Health Examiners.
  • Certifications: Ability to obtain and maintain a valid AZ DPS Fingerprint Clearance Card.
