POSITION SUMMARY
Integrative Emergency Services is in search of a dedicated Care Coordinator with a background in Medical Assisting or Licensed Vocational Nursing. The Care Coordinator plays a vital role in executing both administrative and clinical responsibilities under the supervision of a licensed healthcare professional. This position is essential in enhancing communication between patients and healthcare providers to ensure exceptional patient care.

This role primarily focuses on facilitating care for patients, ensuring they receive the support they need throughout their healthcare journey.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are essential to the role, and additional responsibilities may be assigned as necessary:
1. Regularly assess assigned patients to conduct timely and appropriate care outreach.
2. Schedule routine telehealth appointments with healthcare providers.
3. Liaise with providers, including specialists involved in patient care.
4. Assist in obtaining patient medical records.
5. Develop and maintain patient-centered care plans in accordance with provider directives.
6. Ensure care plans are communicated effectively to patients and their families, adhering to relevant guidelines.
7. Provide education to patients and families regarding their care.
8. Maintain compliance with healthcare management guidelines.
9. Support the enrollment of new patients into care programs.
10. Collaborate with interdisciplinary teams, including social workers and home health providers, to address patient needs.
11. Report any discrepancies or issues to the supervisor promptly.
12. Safeguard the confidentiality of medical records and patient health information in line with privacy standards.
13. Address immediate patient concerns and follow up on inquiries, requests, and complaints, escalating as necessary.
14. Occasionally provide guidance and training to other staff members.

QUALIFICATIONS
To excel in this position, candidates must demonstrate the following knowledge, skills, and abilities:
- Strong analytical, organizational, and communication skills, both verbal and written.
- Attention to detail and the ability to manage multiple priorities effectively.
- Proficiency in customer service and teamwork.
- Familiarity with MS Office applications and internet navigation.
- Ability to communicate fluently in English, with additional language skills being a plus.
- Knowledge of HIPAA regulations and commitment to maintaining confidentiality.
- Compassionate listening skills to support a diverse patient population.

EDUCATION / EXPERIENCE
Required:
- Minimum of 2 years of experience in health education, patient navigation, or a similar healthcare assistance role.
- High School Diploma or GED.
- Valid driver’s license and reliable transportation.
Preferred:
- Certification in Medical Assisting.
- Active nursing license in good standing.
- Associate’s Degree.
- Bilingual capabilities (Vietnamese, Chinese, Arabic, and/or Spanish alongside English).

PHYSICAL DEMANDS
The physical requirements of this role include:
- Ability to see at various distances and adjust focus as needed.
- Regularly required to stand, sit, talk, hear, and use hands for computer and telephone tasks.
- Capability to perform typical office activities, including light to moderate lifting and fine hand manipulation.

WORK ENVIRONMENT
The work environment is characterized by a low noise level and is generally free from hazardous conditions. The organization maintains a smoke-free workplace in compliance with federal regulations.

EXPECTED PAY
Compensation for this position ranges from $18 to $25 per hour.
